FAAN Grandparent Awards
Do you know a grandparent who has done an outstanding job caring or advocating for children with food allergies? The contribution may have involved a single significant event or an accumulation of effort over a period of time. Here is your opportunity to show how much you appreciate their support. Nominate the grandparent for a FAAN Grandparent Award.
FAAN members and supporters have the opportunity to nominate grandparents who have gone above and beyond the call of duty to make a difference in the lives of children with food allergies. FAAN Grandparent Award recipients are recognized for their involvement in increasing awareness and education associated with food allergies.
The winners of the FAAN Grandparent Award will be announced at FAAN’s annual Food Allergy Conferences in 2010.
How to Make a Nomination
To make a nomination for the FAAN Grandparent Award, you must complete a nomination form and respond to questions following the instructions below. You are limited to one nomination.
All nomination forms and question responses must be typed. Nominations must be postmarked, faxed, or e-mailed by 11:59 p.m. ET, Friday, Jan. 15, 2010.
The nomination form requires Adobe Reader version 7.0 or higher is required; available free at www.adobe.com.
